Prior to your appointment you will receive an email with the following questions:
- Have you been diagnosed with COVID 19 or waiting for lab results from a COVID 19 test?
- Have you been in contact with a suspected, probable or confirmed case of COVID 19?
- Do you have any symptoms of COVID 19, even if mild?
